Ordering repeat prescriptions

 YOU NEED TO REGISTER FOR ONLINE ACCESS TO REQUEST REPEAT PRESCRIPTIONS.

You will need to register for the new system in person at the surgery bringing photo ID (Passport or Driving Licence) and a utility bill showing your current address.

Please complete the application form which can be found here.

You should bring this form with you to the surgery, together with your identity documentation.
